Establish Group Insurance Applicant & Enrolling Process 

Labor Insurance:

1. Online Application: enter Bureau of the Labor Insurance Ministry of Labor to apply (log in with the person in charge’s Citizen Digital Certificate)

2. Written Application: Print the website’s paper insurance application form (labor insurance, health insurance and labor retirement 3-in-1 form), increased insurance declaration form(labor insurance, health insurance and labor retirement 3-in-1 form), fill in and mail it to each labor insurance bureau to go through the insurance procedures.

Health Insurance:

1.Online Application: enter National Health Insurance Administration to apply (log in with the person in charge’s health insurance card or Citizen Digital Certificate)

2.Written Application: print out the website‘s paper-based National Health Insurance Group Insurance Applicant Application, and fill in the National Health Insurance Enrollment Application

Mail it to each divisional business group of the National Health Insurance Department to go through the insurance procedures.
